How Mt Tam Sport & Spine different from everybody else?
Our patients and athletes come to us for many reasons - a doctor's or friend's referral, our convenient Mill Valley location, or our personalized treatment, to name a few. They return because of the Mt Tam Sport & Spine experience - the care they receive, the open friendly environment, the fun they have, and the results they achieve.
Do you take my insurance?
Our goal is to provide you with the best personalized health care at an affordable price. We are considered an out-of-network clinic therefore, if you have out-of-network coverage you may be able to get reimbursed for your care. This means we do not directly bill your insurance company, but we are happy to provide you with an itemized receipt/”superbill” of the services provided to you. This will assist you with getting reimbursed for care.
We feel that eliminating billing directly to insurance companies, we are able to spend more time with our patients. This cultivates an environment where we can provide better quality care, more efficient treatments, and faster recovery times for our patients. Everybody wins!
What is out-of-network care?
Any provider who is not contracted with your insurance is considered out-of-network. Some insurances do not cover out-of-network care. Please be sure to confirm this before seeking out-of-network care, especially if you plan on submitting for reimbursement.
How do I know if my insurance has out-of-network benefits?
Some insurance cards have your in-network and out-of-network coverages stated on the front of your insurance card. If your insurance card does not have it indicated, you will have to call your insurance company. You can find customer service phone numbers on the back of your card.
Here are a few recommended questions to ask your insurance company:
Will my plan cover out-of-network care?
If so, what is the coverage?
How many visits do I get a year?
Are the visits combined for PT and Chiropractic or separate? (PT visits in other clinics may also count against your Chiropractic benefit)
Is my coverage a plan year or calendar year?
What documentation is required for reimbursement?
Is there a deadline for submission for reimbursement?
Do I need a referral? (you do NOT need one to see us, but your insurance may require it)
Do I have a deductible for out-of-network services?
If yes, has it been met?
Do I need pre-approval for out-of-network providers?
How do I get my superbill / itemized receipt?
We will provide you a ”superbill” upon request. It will be sent via email.
How do I go about getting reimbursed for my care?
After you have confirmed that you have Chiropractic out-of-network coverage, you will send your itemized receipt, as well as any other documentation your insurance company requires, via the “claims” address located on the back of your insurance card.
Do I need a prescription for Chiropractic services?
You do not need a prescription for Chiropractic services.
What if I am a previous patient, but from another clinic?
First off, we are excited you found our clinic and so excited to continue care with you. You can expect the same high quality care from Dr. Rosenberg you have previously experienced.
What is your cancellation policy?
In consideration of our other patients, we have a 24-hour cancellation policy. Please call or email by noon the business day prior to your appointment to avoid being charged. The fee is the same as the cost of the visit you missed.
What methods of payment do you accept?
We accept cash, debit cards, credit cards, checks, mobile payments such as Apple Pay and others, Medical Reimbursement Accounts (MRA), Flexible Spending Accounts (FSA) and Health Savings Account (HSA) cards.
When do I pay for my care?
Payment in full is required at the end of each visit or prior to beginning package care.
How do I find Mt Tam Sport & Spine?
We are located in the heart of Mill Valley within the Thoroughbred Fitness Studio, right next to Tam High School. Our address is 600 Miller Ave, Mill Valley, CA 94941. Enter up the stairs on the north (right) side of the building.
Do you have parking?
Yes! We have free parking in our lot.
What kind of amenities does your clinic provide?
For your convenience we offer a changing area and lockers, showers, towels, and shower products.
